I think you leak essential nutrients, its the new growth of the leave and its having problems to built properly. Holes are very extreme, but the pattern doesnt look like its from insects. Also the leaf around the holes is dead dry, so its the plant itself. Not an insect

To me she looks like she has a magnesium deficiency. I would suggest adding a bit of cal/mag to your watering.
