Root rot or pH imbalance?

This blueberry muffin (f) plant was doing well until last week when it started deteriorating. Its grown on a mix of cocopeat, vermicompost, perlite mix and fed on compost tea and cal-mag mix. I might put it down eventually but I checked the pH and it was 5!

She is a goner..........a pH of 5 will do that.
After leaving it for so long already and letting it get this bad, I doubt there is anything you can do to help this plant.
