Is this soil ? If yes - its overwatering, girl has to have wet/dry cycle. You have to lift pot, when its easy to lift you water , otherwise problems will go deeper. If its coco, could be light nitrogen toxicity aka food too strong. To me it looks more like first option from look, but i covered other possibility too.
