future reference -- put some perlite or similar into that substrate. lack of o2 doesn't help the roots grow.
just a blurry pic, but sometimes a seedling just calls it quits if the environment isn't conducive... did the substrate dry out? were you digging around or otherwise physically dmage it?
best germination methods are the simplest. plant seeds and get out of the way. if in a cold climate, a heat mat is worth it, but get one with a thermostat probe so you aren't guessing about resulting substrate temperature. A 76-80F substrate will make for more consistent sprout times, but not necessarily more successful... less of an impact in that regard compared to consistency.
