Anyone ever have a seedling come out with 3 Cotyledons?

Yes. It may exhibit whorled phyllotaxy throughout its life, with 3 branches per node. My avatar picture shows how its first true leaves might appear. The characteristic makes for an attractive plant, in my opinion. If you take it all the way to harvest, you’ll find the buds and bud arrangements are aesthetically interesting.
Assuming it maintains whorled phyllotaxy the whole way, each set of 3 branches will emerge in opposite arrangement than the one below, like the points of a triangle, followed by the points of an upside-down triangle, and so on.

Plenty over the last forty odd years of growing..............most turned out duds.........either hermies or bad flower development.
Thse days, they are an automatic cull for me.
You could keep it as a "curio" if you have lots of room, but I would not consider keeping it as a main crop plant or if space was limited.

I’ve had it, usually sorts itself out by the 5th node. Sometimes you’ll get a flat stem where the plant thinks it’s a
“Siamese twin”
You can also have something called polyploid some genetics are more prone to have thing like this.
Shouldn’t effect the grow at all just an uncommon genetic defect.
You can also have seeds with 2 sprouts in them actual twin.

This mutation should not cause a problem outside of maybe a bit more pruning if it gets congested. It may stop popping three leaves/branches per node as it matures, too.
I think bandit's encounter with this mutation may have coincided with something else more severely wrong with his plant. This mutation usually isn't a problem except for the over-growth potential. Crowding a canopy doesn't help.

Hi, I had a seedling like that too, and it was the first and last time I let such a seed grow. Let a new seed germinate. I only had problems, no proper buds. It's your decision what you do. Good luck 🍀
