did you by chance over heat the plants about week 4 is when I would bet you stressed it out . from the pictures I would say it got to hot and didn't get water or fert burn and root damage . the plant freaked out trying to live and pass on its genes . this is one the most common and easy ways to create a hermaphrodite over heating and no water . even plants that are most cases stable will try to survive if feel they are in trouble and nature will due what it does best find a way to live in any condition . if environment is favorable the cell will reproduce naturally if the environment is not stable the cell will favor immortality and try to produce on its own
