It is kinda unlikely that the cotyledon just fell off, especially at that stage.
So it was damaged by something, either excess moisture, handling or a bug, if I had to guess.
But since it has its first leaves, it might be okay....

Do u think its safe to use pond water for watering?Theres a pond near me(man made) with carp in it, Its connected to a stream which goes through the woods. Is there a way I could test the water if its safe, mabey some testing kit?

Why so small?What could cause them to stay small and grow slow. I can think of the below anything else to consider:
1. soil is 40% ordinary potting soil, 30% coco coir and 30% light.
2. light. 1st week was under TL, didn't have grow light yet. Then under 80%
