I've seen one that repeatedly does this. 1 of 100s, so it seems to be rare. a twin wouldn't be fused together. if you didn't top it or accidentally damage a terminal bud, then it's just an odd mutation/growth pattern. E.g. accidentally dripping some fertilizer on a terminal bud might kill the growth tip and cause the 2 axial bods to grow out just like topping the plant would. This could happen from accidental physical damage too.
