drip anything on it? tear anything while removing a seed shell? if nothign physical happened, just bad luck with genetics.
it 'can' produce another growth tip, but it will take a long time. It won't keep up with the rest of your grow. if not inconvenient, you can let it tag along, but if filling in a scrog or something it makes shifting things around to compensate difficult. no scrog? get some risers/pot elevators so you can keep it level if it does stat growing.
i'd throw it out, but probably not a lot of effort to tug it along.
Since you had germination issues, consider the substrate you use too. was pH off? was it hot? did it have worm casting in it from teh start? maybe use a smaller seedling pot with just washed/buffered coco and hold off on the amended stuff until first transplant. if some concentration of ferts was lodged between seed shell and coty/growth tip, that can cause some oddities like burning off a growth tip - no different than dripping some fertilizer on it, really.. or worse since it won't evaporate as quickly when absorbed into a solid substrate.
