Does anyone know what kind of mutation this is? How/why do these occur? Unstable genetics, environmentals, feeding, etc.?

There is no mutation, but the one your referring to is a polyplody I believe. Go easier on nitrogen in flowering. Nitrogen can hinder bud development in bulking. They want a lot of potassium, and phosphorus with very little nitrogen in bulking. I can tell by the darker shade of green. 👍🏻🌱

ah, leaves / axillary buds on three axes? just genetics. tri-leaf mutation. heh it did seem like a shit-ton of leaves though i would disagree about how much N should be adjusted in bloom.. i only drop mine about 15-20% ppm concentration in bloom (130ish to less than 110). i did have minor very slowly progressing n-tox in late bloom before adding that wrinkle. I never did it because it was assumed. I reacted to plant behaviour. it was nothing major that would cause a large cola to result in a small cola. Seen numerous plants with burnt to crisp canopies produce large donks. It's mostly genetics. The wavelength of light, temps, rh, nutes will not turn golf ball nugs into donks.. it won't change medium-sized colas into donks. but do have "some" level of impact relative to what genetics dictates.

Like said, in most cases it's random. Can be PH, cold weather, temperature changes.. but i have to say, this happened to me one time, with an excess of light at some point of the colas, with "not appropriate" LED lenses, was pre-installed with the equipment. So, that doesn't seems like that in your case, but for me, the solution was to put a little distance from the LED, and delete this "bad" penetration lenses who was included in the device. Mutations are most of the time random but in some cases it might be triggered by cold weather or temperature fluctuations in general. I love to see the random mutations because that's what makes each plant even more different. Thanks for posting this and good luck on your grow
